Woodcrest Rehabilitation & Residential a facility located in Flushing, NY is seeking Full-time Social Worker (BSW or MSW).
- Long Term Care/ Subacute experience preferred.
- Attend and participate in interdisciplinary care plan meetings.
- Complete MDS while developing and maintaining individual care plans.
- Interview and access residents’ needs during the admission interview process.
- Work with the team to coordinate discharges.
- Work with residents to maintain psychosocial well-being.
